Handeling 1-2524

 

Algemene infectiepreventie bij neutropenie

  • PDD volgens lokaal gebruik conform de conditioneringsschema’s.

 

Let op: alleen van toepassing bij een conditionering die gepaard gaat met langdurige neutropenie (> 7 dagen). Inventarisatiekweken worden als eerste afgenomen 48 uur na start van de antibiotica en daarna éénmaal per week van keel en anus gekweekt op enterobacterales / pseudomonas en gisten.

Streptococcenprofylaxe

  • Feneticilline 4 dd 250 mg oraal.
  • Penicilline 4 dd 106 IE i.v.

 

Bij conditioneringsschema’s die gepaard gaan met mucositis, conform de conditioneringsschema’s.

Pneumocystispneumonie (PCP) profylaxe

  • Co-trimoxazol: 1 dd 480 mg p.o. vanaf dag +1.

 

Tweede keus

  • Pentamidine verneveling 1 maal per maand 300 mg per inhalator, voorafgegaan door salbutamol 2,5 mg per verneveling.

Derde keus

  • Atovaquon 1 dd 1500 mg p.o. (innemen met vetrijk voedsel).

 

Wegens gecombineerde bescherming tegen PCP, toxoplasma en pneumokokken is co-trimoxazol eerste keus.  Contra-indicaties kritisch bekijken.

 

NB Co-trimoxazol is de enige goede profylaxe tegen toxoplasmose.

Wees, zonder co-trimoxazol, bij GvHD bij toxoplasma seropositieve patiënten alert op toxoplasmose reactivatie.

Pneumococcen profylaxe indien geen co-trimoxazol wordt gegeven

  • Feniticilline 2 dd 250 mg of Azitromycine 1 dd 250 mg drie dagen per week.

Herpes simplex en Varicella zoster profylaxe

  • Valaciclovir 2 dd 500 mg p.o. vanaf dag +1.
  • Aciclovir 2 dd 500 mg i.v.

 

Duur: Valaciclovir en co-trimoxazol worden beiden gecontinueerd tot revaccinaties compleet zijn en mits CD4 getal dan > 200/ μl (0.2 x 109/l). Z.n. CD4 getal na 4 maanden herhalen. Zie voor de (duur van) toepassing van co-trimoxazol als Pneumokokkenprofylaxe en valaciclovir als Varicella zoster profylaxe ook de revaccinatierichtlijn.

Aspergillus profylaxe

Indicaties:

  • Primaire profylaxe: 
    • Bij GvHD waarvoor prednison > 1 mg/kg/dag.
      Duur: tot de prednison < 20mg 1dd is afgebouwd.
    • Bij geschatte / verwachtte neutropenie duur van > 30 dagen.
      Duur: tot resolutie neutropenie (≥ 0.5 x 109/l).

 

  • Secundaire profylaxe:
    • Na eerdere possible, probable of proven aspergillose én
    • Nieuwe episode hoog risico neutropenie (≥ 7 dagen, ook niet-chemotherapie gerelateerd).

 

Duur: tot (twee weken) na resolutie neutropenie (≥ 0.5).

  • 1e keus: Posaconazol 1 dd 300 mg (dag 1: 2 dd 300 mg) tabletten.
  • 2e keus: Voriconazol 2 dd 200 mg (dag 1:  2 dd 400 mg).

 

Therapeutic drug monitoring (TDM) voor profylactische behandeling:

Posaconazol dalspiegel > 0,7, mg/L, voriconazol dalspiegel1, 5-6.

 

NB: voor secundaire profylaxe dient rekening gehouden te worden met azool resistentie van eerdere infectie.

NB: cave interacties tussen azolgebruik en ciclosporine, tacrolimus en sirolimus.

 

Verantwoording: er is geen adequaat prospectief onderzoek voor primaire of secundaire profylaxe, behoudens bij GvHD. De adviezen binnen de SWAB richtlijn zijn niet toegespitst op de verscheidenheid van conditionerings en transplantatieschema’s met hun eigen risicoprofiel, waardoor gekozen is voor bovenstaande advies [1-4].

Immunoglobulines (controle en profylaxe)

Bij hypo / agammaglobulinemie én recidiverende luchtweg infecties dient profylactische behandeling te worden overwogen [5-7]:

 

Eerste keus is profylactische antibiotica

  • eerste keus:
    • Co-trimoxazol 1 dd 960 mg.
  • tweede keus:
    • Claritromcycine 1 dd 500 mg.
    • Azitromycine 3 keer per week 250 mg.

 

Indien dit niet effectief of mogelijk is: overweeg start IVIG:

Aanvangsdosis :   0.3 g/kg.

Onderhoud       :   0.2 g/kg elke 4 weken, streef naar dalspiegel IgG ≥ 5 g/L. 

 

Evaluatie van klinisch effect na 6 maanden tot 1 jaar.
